Mission Statement
The mission of the office of Vital Statistics is to maintain a uniform system for registration and a permanent and continuous index for all birth, death, and marriage records which occur in the Township of Washington. This office works hand-in-hand with area hospitals, funeral homes, and the NJ State Registrar's Office to carry out the registration process and provide a courteous and customer friendly service to the public.

TO APPLY FOR A MARRIAGE LICENSE

If you have downloaded the form it may be completed prior to your visit, or you may obtain and complete the form at the Clerk's Office. The Front Page may be completed prior; however the back page is to be completed in the presence of the Registrar during your visit.

The following items must be provided:

  1.  Click here to print the marriage application form from our website.  Application must be  double-sided and only one (1) page.
  2.  Photocopies of both the bride and the groom’s drivers’ licenses showing current address.
  3. If anyone has been previously married, a copy of the divorce decree is helpful.
  4. Cash, check or money order, in the amount of $28, payable to the Township of Washington. 
  5. You must have a witness over the age of 18 who knows you personally.

The bride, groom and one witness must appear for the signing. Marriage license are valid for 30 days from the date of issue.

*Marriage licenses must be held for 72 hours as per the state guidelines. Please allow ample time from the time your application is accepted, to the actual date of the ceremony.

** The State of New Jersey has waived the 72-hour waiting period during the pandemic phase only! **
 
 

TO APPLY FOR A COPY OF A BIRTH, MARRIAGE OR DEATH CERTIFICATE

In order to request a Vital Record (copy of a birth, marriage or death certificate) you may do so  in person or via the Washington Township Dropbox located in our parking lot.

  1. Click here to print the request for vital record.
  2. Be sure to complete the form, indicate the number of copies you are requesting, sign and date.
  3. The cost is $20 for the first copy, $5 for each additional copy. Cash, check or money order payable to “Township of Washington”.
  4. If you are mailing the form and payment or using the dropbox, you must include a copy of your driver’s license showing current address and a self-addressed stamped envelope. If the vital record is not for you or your child, you must provide proof of relationship.
  5. Place all documents in the dropbox in the parking lot of the municipal building or mail to
